I have recently subscribed to these mailing lists and need some help in 
finding or writing some header files for use with ultramid.  I recently 
downloaded the GUS SDK and have been paying a lot of interest in 
ultramid.doc.  However to compile any of the example programs a few 
header files like ultramid.h and midi.h are needed.  It is possible to 
construct a copy of ultramid.h from the documentation in the sense of 
satisfying compiler errors but i can't get anything to run.  I get a 
few non-portable pointer conversion warnings or suspicious warnings as 
I have not figured out the correct types for all of the header file.  
I'm sure there is also a lot missing.  Can anyone help me or point me 
to a ftp site.  I have looked but been unsuccessful.  Anything to do with 
ultramid would help.

I've got a GUS and a SB pro working fine together in my system.. The SB 
is at DMA 1, IRQ 7, Port 220, and the GUS is at DMA 7, IRQ 11, Port 250.  
Try setting your GUS setting manually in your autoexec.bat and 
system.ini file.  I believe the GUS setup program trys to set up the 
GUS for SB emulation, so that may be where your problems are coming 
from.
